Case Managers work with a limited caseload (8-10 young adults at a time) doing intensive and clinically focused case management services, including treatment plans, safety planning, functional analysis, and supportive group and individual supervision. The LifeSet program is an evidence based case management model that is being implemented at the Y Social Impact Center in King County in partnership with Youth Villages. We serve young adults 18-22 years old throughout King County.
