    Are you passionate about putting an end to childhood obesity? The Matt Griffin is hiring two Spanish Actively Changing Together (ACT!) program facilitators. The YMCA ACT! program is a 12 week program for youth 8-14 years old and their parents promoting healthy nutrition, activity and lifestyles developed in partnership with providers and professionals at Seattle Children's Hospital and the Y of Greater Seattle. One position will lead the physical activity portion and the other will lead a nutrition lesson and food demonstration. Both positions provide support and guidance to participants in the YMCA’s chronic disease prevention programs.   Implements standard curriculum while delivering assigned programs and ensures quality operation consistent with YMCA guidelines.   Has strong understanding of chronic disease illnesses and the impact they have on individuals, families and community.  Understands the Health Seeker target market and provides motivational coaching to members participating in ongoing Healthy Living programs.

     

    EFFECT ON END RESULTS

     

    The Chronic Disease Coach leads and facilitates YMCA Chronic Disease programs for branches within the YMCA of Greater Seattle, creating lasting and positive impact on participants, branches and the community.

     

    Responsibilities

    PRINCIPAL ACTIVITIES

    1. Administers assigned Healthy Living Program ACT! following Association standards and guidelines.
    3. Works with directors to recruit participants and deliver classes.
    4. Assists with special events and seasonal wellness programs for members.
    5. Assists with marketing and retention activities regionally to create robust classes with high enrollment.  May conduct outreach with local organizations and providers for program recruitment.

    9. Maintains records related to program including monitoring prospects and participants.

    10. Plans, facilitates and instructs individuals, groups and/or classes.
    11. Encourages member involvement and identifies potential volunteers.
    12. Conveys basic information on Healthy Living program areas.

    13. Enrollment numbers for the classes are consistent with program standards.

    14. Program targets are reached.

    15. Other duties as assigned.

    POSITION REQUIREMENTS

    1.    Current state approved first aid certification.

    2.    Current state approved CPR for the Professional Rescuer certification.

    3.    Two years or more of college education in a related field or equivalent preferred.

    4.    One or more years of related experience preferred.

    5.    YMCA Healthy Lifestyles Principles certification*

    6.    Listen First Level I* and II** training

    7.    Building Community in Small Groups training***

    8.    Ability to respond to safety and emergency situations.

    9.    Working knowledge of preventative health and fitness programs and philosophy

    10. Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful. 

    11. Must attend and receive passing grade in YMCA of the USA or other training courses related to program delivery (i.e. LiveSTRONG, Pedaling for Parkinson’s, Living Tobacco Free, Cardiac Wellness, etc.)  prior to class

    12. Community Health Worker or equivalent preferred.

     

    * Within 30 days of employment or first available training.

    ** Within 90 days of employment or first available training.

    ***Within 18-24 months of employment.
